At Sioux City Family Dentistry, we believe quality dental care should be accessible to everyone. Our Smile Savers Plan is an in-house dental discount membership designed for patients without dental insurance, helping you save on preventive care and additional treatments throughout the year.

Membership Options

Single

$395/year

Includes:

  • 2 regular cleanings
  • 2 periodic exams
  • 1 set of necessary X-rays
  • 1 fluoride treatment

Dual

$740/year

Available for spouses.

Additional family member pricing:

  • Children 13 & under — $250 each
  • Children 14–19 — $345 each

Periodontal Maintenance

$690/year

Includes:

  • 4 periodontal maintenance visits (periodontal maintenance cleaning and 1 fluoride treatment)
  • 2 doctor exams
  • 1 set of necessary X-rays
  • 1 fluoride treatment per visit

Plus:

  • 10% off SRP treatment when paid prior to treatment

Plan Benefits

Diagnostic & X-Rays

Covered at 100%:

  • Comprehensive exams – (new patient)
  • Periodic exams (2 per year)
  • Limited oral exam (1 per year)
  • 1 set of bitewing X-rays per year and up to 3 periapicals
  • Full mouth series or panoramic X-rays (1 every 3 years)

Preventive Care

Covered at 100%:

  • Adult & child cleanings (2 per year)
  • Fluoride varnish (1 per year)

Additional savings:

  • 10% off sealants
  • 10% off crowns, fillings, veneers, periodontics, and other procedures

How to Enroll

  1. Fill out an enrollment form at our office.
  2. Submit payment at the time of enrollment

Important Plan Information

  • This is a 12-month membership plan
  • Membership fees are non-refundable
  • Payment is due at the time services are provided
  • No yearly maximums
  • Membership must remain continuous during the full treatment period to receive discounted services.
  • Valid only at Sioux City Family Dentistry.
